The following is a checklist of items you will need to download and completely fill-out in order to complete the application process.

Special Services Form
Copy of student's birth certificate
Copy of student's Baptismal certificate (if applicable)
Kindergarten Parent Questionnaire
Kindergarten Teacher Questionnaire
1st Grade Parent Questionnaire
1st Grade Teacher Questionnaire
Two recommendations from current teachers and/or principal (required for grades 2-8)
Copy of last report card (Grades 1-8)
Copy of all standardized test scores:
